1) All we need to do is to set a Start Date (See pictures for reference) the calendar of checklist will be automatically adjusted to next 4 weeks from your start date.

2) Enter the Tasks , there is place for 30 tasks however we can enhance or reduce.

3) Select the Results based upon actual happenings. T for True (Green), F for False (Red) and OFF for Not Planned or Off days (Gray)

There is place for Supervisors Name and Signatory!
