I can also add a contact form for one of the pages. You must already have well layered photoshop files that I will used to create the theme. With this hourlie, you will be allowed up to 2 revisions which were not included in the original design.
This hourlie is for non responsive designs and not included special features. For responsive design please add $49. For extra features, page templates, and content, please contact me for a quote.
